Overview
- Provide compassionate care to hospice patients, ensuring their physical and psychosocial needs are met.
- Develop and implement individualized nursing care plans for patients in end-of-life stages.
- Collaborate with interdisciplinary teams to deliver comprehensive hospice care.
- Supervise and evaluate home health aides and personal care workers in patients' homes.
- Request and manage physician orders for adjustments to patient care plans.
- Ensure accurate documentation of clinical data and patient progress.
- Educate patients and families on hospice care processes and expectations.
- Maintain compliance with healthcare regulations and standards in hospice care.
Key Responsibilities & Duties
- Conduct thorough physical and psychosocial assessments for hospice patients.
- Develop and implement nursing care plans tailored to individual patient needs.
- Collaborate with physicians to adjust care plans as necessary.

Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent required.
- Registered Nurse (RN) with a minimum of 2 years of clinical experience.

- Strong communication, teaching, and listening skills essential.
- Ability to work collaboratively within an interdisciplinary team environment.
- Proficiency in clinical data documentation and management.
