Overview

  • Serve as a vital link between healthcare providers and the community, focusing on HIV-related education and support initiatives.
  • Enhance community engagement by fostering relationships with organizations and individuals affected by HIV.
  • Advocate for patients, addressing barriers to care and promoting access to essential services.
  • Collaborate with healthcare teams to ensure comprehensive patient support and resource allocation.
  • Conduct outreach programs to educate communities about HIV prevention and support services.

Key Responsibilities & Duties

  • Provide direct support to individuals living with HIV, connecting them to care and resources.
  • Conduct community outreach and educational initiatives to raise awareness about HIV prevention and treatment.
  • Build and maintain relationships with community organizations to enhance collaborative efforts.

Job Requirements

  • Associate of Applied Science (AAS) degree or equivalent educational background.
  • Minimum of 1 year of experience in community outreach or healthcare; 4 years preferred.
  • Strong knowledge of HIV prevention, treatment, and support services.
  • Excellent interpersonal and organizational skills to manage diverse responsibilities.
  • Bilingual proficiency in Haitian Creole is preferred to enhance communication.
  • Ability to work on-site and collaborate effectively with healthcare teams.
  • Passion for advocacy and commitment to improving community health outcomes.
  • Familiarity with record-keeping and documentation practices.
