Overview
- Provide high-quality nursing care to patients in Manhattan, ensuring their well-being and recovery.
- Educate patients and families on health prevention and healthy habits.

- Perform home health services, including admissions, re-certifications, and discharges.
- Utilize OASIS D1 assessments for patient evaluations and documentation.
- Ensure compliance with health and safety guidelines and regulations.

Key Responsibilities & Duties
- Report daily to the Clinical Manager regarding patient care and progress.
- Conduct home visits to assigned caseloads, ensuring comprehensive care delivery.
- Develop and coordinate physician care plans with healthcare professionals.
- Perform OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
- Ensure all home care services meet agency and regulatory standards.
- Provide compassionate and professional nursing care with minimal supervision.
- Document patient care activities within the required timeframe.
- Promote patient health and recovery through effective care practices.
Job Requirements
- Valid Nursing license and proven experience as a registered nurse.
- Associate of Applied Science (AAS) in Nursing or equivalent.
- Experience in acute care, hospital settings, or home care.
- Proficiency in comprehensive OASIS assessments and documentation.
- Ability to drive and access a vehicle for patient visits.
- Familiarity with Homecare Homebase (HCHB) software is advantageous.
- Strong commitment to providing culturally diverse and high-quality care.
