Overview
- Provide high-quality home health nursing care to patients in the Bronx area.
- Educate patients and families on health prevention and wellness practices.
- Collaborate with healthcare professionals to develop and implement care plans.
- Utilize OASIS D1 assessment for patient admissions, transfers, and discharges.
- Document care and assessments accurately using provided tablet technology.
- Ensure compliance with health regulations and agency standards.
- Participate in orientation and training sessions to maintain proficiency.
- Support the agency's mission to deliver culturally competent care.
Key Responsibilities & Duties
- Conduct home visits to assigned patients, performing assessments and providing care.
- Develop and update Physician Plans of Care in collaboration with the healthcare team.
- Complete comprehensive OASIS assessments for patient care transitions.
- Maintain accurate and timely documentation of care activities.
- Provide patient education to promote health and prevent illness.
- Ensure adherence to Department of Health regulations and standards.
- Participate in regular communication with the Clinical Manager.
- Utilize agency-provided technology for efficient care delivery and documentation.
Job Requirements
- Valid Registered Nurse license with proven experience in nursing care.
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Experience in pediatric care and acute hospital or home care settings.
- Proficiency in completing OASIS assessments for patient care transitions.
- Ability to drive and access a reliable vehicle for patient visits.
- Familiarity with Homecare Homebase (HCHB) software is advantageous.
- Strong commitment to delivering culturally competent and high-quality care.
- Excellent communication and organizational skills to manage patient caseloads.
