Key Responsibilities & Duties

  • Conduct home health visits, including admissions, re-certifications, and discharges.
  • Develop physician care plans in collaboration with healthcare teams.
  • Complete OASIS D1 assessments for patient admissions and transitions.
  • Ensure timely documentation using provided tablets within 24-48 hours.
  • Provide culturally sensitive care that aligns with agency standards.
  • Report daily to the Clinical Manager regarding patient care progress.
  • Maintain high-quality care standards in accordance with regulations.
  • Support patient recovery by fostering a compassionate and professional environment.

Job Requirements

  • Associate of Applied Science (AAS) degree in Nursing.
  • Valid Nursing license with proven experience as a Registered Nurse.
  • Experience in pediatric care and acute hospital or home care settings.
  • Proficiency in completing comprehensive OASIS assessments.
  • Familiarity with Homecare Homebase (HCHB) software is advantageous.
  • Ability to drive and access a reliable vehicle for patient visits.
  • Strong commitment to patient care and adherence to health regulations.
  • Excellent communication and organizational skills.
