Key Responsibilities & Duties
- Conduct home health visits, including admissions, re-certifications, and discharges.
- Develop physician care plans in collaboration with healthcare teams.
- Complete OASIS D1 assessments for patient admissions and transitions.
- Ensure timely documentation using provided tablets within 24-48 hours.
- Provide culturally sensitive care that aligns with agency standards.
- Report daily to the Clinical Manager regarding patient care progress.
- Maintain high-quality care standards in accordance with regulations.
- Support patient recovery by fostering a compassionate and professional environment.
Job Requirements
- Associate of Applied Science (AAS) degree in Nursing.
- Valid Nursing license with proven experience as a Registered Nurse.
- Experience in pediatric care and acute hospital or home care settings.
- Proficiency in completing comprehensive OASIS assessments.
- Familiarity with Homecare Homebase (HCHB) software is advantageous.
- Ability to drive and access a reliable vehicle for patient visits.
- Strong commitment to patient care and adherence to health regulations.
