Nurse Practitioner

Overview

• Provide primary care services to patients in an on-site setting.
• Diagnose and manage acute and chronic conditions effectively.
• Prescribe medications and handle refill requests as necessary.
• Communicate with patients, families, and healthcare personnel.
• Order and interpret diagnostic tests to inform treatment plans.
• Document patient histories and treatments accurately in EMR systems.
• Work flexible hours with options for part-time or full-time schedules.

Key Responsibilities & Duties

• Deliver in-person primary care services to patients in accordance with protocols.
• Diagnose, treat, and manage patient conditions effectively.
• Prescribe medications and manage prescription refills responsibly.
• Collaborate with healthcare teams, including case managers and social workers.
• Order diagnostic tests and interpret results to guide treatment.
• Maintain accurate and timely documentation in electronic medical records.
• Ensure compliance with Medicare/Medicaid provider requirements.
• Participate in patient care coordination and communication efforts.

Job Requirements

• Bachelor of Science in Nursing (BSN) required.
• Active unrestricted New York state Nurse Practitioner license.
• Minimum two years of clinical experience preferred.
• DEA certification and PECOS enrollment required.
• Knowledge of eClinicalWorks EMR system preferred.
• Medicare/Medicaid approved provider status essential.
• Coalition for Affordable Quality Healthcare (CAQH) setup required.
• Basic Cardiac Life Support (BCLS) certification necessary.
