Job Description

Overview

  • Provide primary care services to patients in an on-site setting.
  • Diagnose, treat, and manage acute and chronic conditions effectively.
  • Prescribe and refill medications as per patient requirements.
  • Communicate actively with patients, families, and healthcare personnel.
  • Order and interpret diagnostic tests including imaging and laboratory work.
  • Document patient histories and treatments in electronic medical records.
  • Work part-time or full-time with flexible scheduling options.
  • Opportunity for additional compensation based on billable visits.

Key Responsibilities & Duties

  • Provide in-person general primary care services to patients.
  • Diagnose and manage both acute and chronic medical conditions.
  • Prescribe medications and handle prescription refills as necessary.
  • Order and interpret diagnostic tests including imaging and lab work.
  • Document patient histories and treatments accurately in EMR systems.
  • Communicate effectively with patients, families, and healthcare teams.
  • Ensure compliance with Medicare/Medicaid and other healthcare regulations.
  • Participate in flexible scheduling options, including floating to nearby sites.

Job Requirements

  • Bachelor of Science in Nursing (BSN) degree required.
  • Active unrestricted New York state Nurse Practitioner license.
  • Minimum two years of clinical experience, preferably in long-term care.
  • DEA certification and Medicare/Medicaid provider approval required.
  • PECOS enrollment and BCLS certification preferred.
  • Knowledge of eClinicalWorks EMR system is advantageous.
  • Ability to work flexible schedules and float to nearby sites.
  • Coalition for Affordable Quality Healthcare (CAQH) setup required.
