Job Description

Overview

  • Provide in-person primary care services to patients, diagnosing and managing acute and chronic conditions.
  • Work part-time Monday, Wednesday, Friday, or full-time with floating to nearby sites.
  • Opportunity to earn additional compensation for exceeding billable visit thresholds.
  • Collaborate with patients, families, specialists, and facility personnel for comprehensive care.
  • Document patient histories and treatments accurately in electronic medical records.
  • Perform diagnostic tests and interpret results to guide treatment plans.
  • Prescribe and refill medications as necessary, ensuring compliance with regulations.
  • Enhance patient care through proactive communication and coordination with healthcare teams.

Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Prescribe medications and handle prescription refills efficiently.
  • Order and interpret diagnostic tests, including imaging and lab work.
  • Maintain accurate and detailed patient records in the EMR system.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Provide patient education and counseling to promote health and wellness.
  • Ensure compliance with Medicare/Medicaid and DEA regulations.
  • Participate in quality improvement initiatives and professional development.

Job Requirements

  • Bachelor of Science in Nursing (BSN) with active New York state NP license.
  • Minimum two years of clinical experience, preferably in long-term care settings.
  • DEA certification and PECOS enrollment required.
  • Knowledge of eClinicalworks and BCLS certification preferred.
  • Medicare/Medicaid approved provider with CAQH setup.
  • Ability to work on-site with flexibility to float to nearby locations.
  • Strong communication and interpersonal skills for patient and team interactions.
