Overview
- Provide in-person primary care services to patients, diagnosing and managing acute and chronic conditions.
- Work part-time Monday, Wednesday, Friday, or full-time with floating to nearby sites.
- Opportunity to earn additional compensation for exceeding billable visit thresholds.
- Collaborate with patients, families, specialists, and facility personnel for comprehensive care.
- Document patient histories and treatments accurately in electronic medical records.
- Perform diagnostic tests and interpret results to guide treatment plans.

- Enhance patient care through proactive communication and coordination with healthcare teams.
Key Responsibilities & Duties
- Diagnose, treat, and manage acute and chronic medical conditions.
- Prescribe medications and handle prescription refills efficiently.
- Order and interpret diagnostic tests, including imaging and lab work.
- Maintain accurate and detailed patient records in the EMR system.
- Communicate effectively with patients, families, and healthcare professionals.
- Provide patient education and counseling to promote health and wellness.
- Ensure compliance with Medicare/Medicaid and DEA regulations.
- Participate in quality improvement initiatives and professional development.
Job Requirements
- Bachelor of Science in Nursing (BSN) with active New York state NP license.
- Minimum two years of clinical experience, preferably in long-term care settings.
- DEA certification and PECOS enrollment required.
- Knowledge of eClinicalworks and BCLS certification preferred.
- Medicare/Medicaid approved provider with CAQH setup.
- Ability to work on-site with flexibility to float to nearby locations.
- Strong communication and interpersonal skills for patient and team interactions.
