Job Description

Overview

  • Provide essential nursing care and medical procedures within the scope of LPN practice.
  • Collaborate with healthcare providers to ensure comprehensive client care.
  • Support integration of physical and behavioral health in the program.
  • Administer prescribed treatments and document adherence to policies.
  • Respond promptly to medical emergencies, providing necessary triage and follow-up.
  • Maintain accurate patient documentation as required by regulations.
  • Participate in quality improvement projects to enhance client outcomes.
  • Ensure patient-centered care, fostering trust and respect.

Key Responsibilities & Duties

  • Administer prescribed medical treatments following doctor’s orders and internal policies.
  • Schedule appointments and manage medical documentation efficiently.
  • Act as liaison between medical providers and the facility for client needs.
  • Perform general nursing care, including vital signs monitoring and medication administration.
  • Conduct phlebotomy and specimen preparation for laboratory analysis.
  • Respond to emergencies, providing immediate care and stabilization.
  • Ensure compliance with documentation standards and regulatory requirements.
  • Assist RN and nursing leadership with tasks within LPN scope.

Job Requirements

  • Active LPN nursing license in New York State.
  • Current CPR certification is mandatory.
  • Proficiency in oral and written communication.
  • Strong organizational and data management skills.
  • Ability to respond swiftly to emergencies and perform necessary protocols.
  • Physical stamina to perform nursing duties effectively.
  • Experience in lifting, transferring, and repositioning clients safely.
