Overview

  • Lead and mentor a team of hospice nurses, ensuring high-quality patient care and professional development.
  • Collaborate with the Hospice Nurse Manager to implement policies and achieve program goals effectively.
  • Balance fieldwork and office responsibilities, with 80% fieldwork and 20% office-based activities.
  • Carry a reduced caseload, providing direct patient care and case management services.
  • Support staff orientation and precepting, ensuring smooth integration and skill development.
  • Promote professional growth among team members through mentorship and training initiatives.
  • Ensure compliance with hospice and palliative care standards and regulations.
  • Work in a dynamic environment with rotating weekend schedules and flexible hours.

Key Responsibilities & Duties

  • Provide leadership and guidance to hospice nursing staff, fostering a collaborative and supportive work environment.
  • Supervise and mentor nursing staff, ensuring adherence to best practices and professional standards.
  • Carry a reduced caseload, delivering patient care and managing cases effectively.
  • Assist in staff orientation and training, enhancing team capabilities and integration.
  • Collaborate with the Hospice Nurse Manager to achieve program objectives and improve service delivery.
  • Perform fieldwork to oversee case management and supervise staff activities.
  • Ensure compliance with hospice care policies, procedures, and regulatory requirements.

Job Requirements

  • Associate of Science in Nursing required; Bachelor of Science in Nursing preferred.
  • Minimum of six years of nursing experience, preferably in hospice care.
  • Supervisory or leadership experience preferred, with strong mentoring capabilities.
  • New York State RN licensure required; CHPN certification preferred or obtained within one year.
  • Proficiency in Microsoft Word, Outlook, and Windows applications.
  • Ability to effectively supervise and mentor hospice nursing staff.
  • Strong communication and organizational skills to manage fieldwork and office responsibilities.
