Overview
- Provide medical case management for clients in permanent supportive housing, focusing on complex medical and psychiatric needs.
- Collaborate with healthcare professionals to optimize client outcomes and ensure comprehensive care coordination.
- Work on-site to deliver health education and advocate for client health goals and preferences.
- Maintain medication administration records, controlled substance logs, and ensure adequate medication supply.
- Assist clients with practical needs, such as researching services and understanding documentation.
- Provide emergency first aid and CPR assistance as required.
- Participate in treatment team meetings and case conferences to share findings and coordinate care plans.
- Contribute to a supportive environment for clients and staff through effective communication and teamwork.
Key Responsibilities & Duties
- Supervise assigned staff and ensure adherence to organizational policies and procedures.
- Link clients' medical and psychosocial needs, making referrals and suggestions for treatment and rehabilitation.
- Conduct health checks and coordinate with nursing leadership to manage client health needs effectively.
- Document client interactions and maintain accurate records of medication administration and controlled substances.
- Provide direct client support, including health education and advocacy for individual health preferences.
- Collaborate with team members to develop and implement client health goals and treatment plans.
- Respond to emergencies and provide immediate medical assistance as needed.
- Assist clients with practical needs, such as navigating services and understanding documentation.
Job Requirements
- Active Licensed Practical Nurse (LPN) certification required.
- Associate of Science (AS) degree in Nursing or related field.
- Minimum of 1 year of experience; 3 years preferred.
- Strong knowledge of medical case management and psychiatric rehabilitation practices.
- Proficiency in maintaining accurate medical records and documentation.
- Ability to provide health education and advocate for client health goals.
