Overview

Key Responsibilities & Duties
- Provide medical care for patients with acute and chronic illnesses, ensuring comprehensive treatment plans.
- Lead care teams to coordinate patient care and achieve health goals effectively.
- Conduct patient assessments, diagnoses, treatments, and health education sessions.
- Engage with hospitalists and transitional care teams for coordinated care management.
- Participate in clinical meetings and risk management programs to ensure compliance.
- Supervise and collaborate with Advanced Practice Practitioners (APPs) within the practice.
- Promote patient health through evidence-based medicine and lifestyle interventions.
- Maintain accurate and concise documentation of patient interactions and care plans.
Job Requirements
- Doctorate of Nursing Practice (DNP) with active State Medical License.
- Board certification and DEA number for controlled substances required.
- Basic Life Support (BLS) certification within 90 days of employment.
- Proficiency in Microsoft Office Suite and electronic medical record systems.
- Strong commitment to patient-centered care and value-based healthcare principles.
- Ability to work onsite during weekdays with no remote or weekend requirements.
- Fluency in English; Spanish proficiency preferred for certain locations.
- Willingness to travel locally for training and occasional regional/national events.
