Job Description

Overview

  • Serve as a Nurse Practitioner providing comprehensive care to patients with acute and chronic conditions.
  • Collaborate with interdisciplinary teams to deliver holistic, patient-centered care in underserved communities.
  • Utilize value-based care principles to enhance patient outcomes and satisfaction.
  • Work onsite during weekdays with no weekend or after-hours requirements.
  • Engage in leadership activities to promote positive culture and team collaboration.
  • Document patient interactions efficiently using technology for streamlined processes.
  • Participate in quality improvement initiatives to enhance care delivery standards.
  • Support the recruitment and onboarding of new providers and patients.

Key Responsibilities & Duties

  • Provide medical care for patients with acute and chronic illnesses, ensuring comprehensive treatment plans.
  • Lead care teams to coordinate patient care and achieve health goals effectively.
  • Conduct patient assessments, diagnoses, treatments, and health education sessions.
  • Engage with hospitalists and transitional care teams for coordinated care management.
  • Participate in clinical meetings and risk management programs to ensure compliance.
  • Supervise and collaborate with Advanced Practice Practitioners (APPs) within the practice.
  • Promote patient health through evidence-based medicine and lifestyle interventions.
  • Maintain accurate and concise documentation of patient interactions and care plans.

Job Requirements

  • Doctorate of Nursing Practice (DNP) with active State Medical License.
  • Board certification and DEA number for controlled substances required.
  • Basic Life Support (BLS) certification within 90 days of employment.
  • Proficiency in Microsoft Office Suite and electronic medical record systems.
  • Strong commitment to patient-centered care and value-based healthcare principles.
  • Ability to work onsite during weekdays with no remote or weekend requirements.
  • Fluency in English; Spanish proficiency preferred for certain locations.
  • Willingness to travel locally for training and occasional regional/national events.
