Job Description

Overview

  • Provide professional rehabilitation therapy to patients in their homes, ensuring individualized, goal-oriented therapy programs aligned with organizational policies and procedures.
  • Perform comprehensive patient evaluations, developing tailored treatment plans with measurable short and long-term goals.
  • Document patient progress, reassessments, and discharge summaries accurately and timely, supporting the need for continued skilled care.
  • Coordinate with physicians, healthcare teams, and resources to ensure effective patient care and service utilization.
  • Supervise Physical Therapy Assistants, ensuring adherence to interventions and regulatory requirements.
  • Educate patients and caregivers through comprehensive, understandable programs tailored to their needs.
  • Maintain productivity requirements and flexibility with job responsibilities, adapting to various patient needs.
  • Participate in discharge planning, addressing patient needs and recommending further services or interventions.
  • Adhere to organizational competencies and standards of behavior, ensuring professional conduct.

Key Responsibilities & Duties

  • Prepare and maintain precise documentation, including progress notes, reassessments, and discharge summaries.
  • Coordinate with healthcare teams and resources to optimize patient care and service delivery.
  • Supervise and assess Physical Therapy Assistants, ensuring compliance with interventions and regulations.
  • Educate patients and caregivers on tailored home programs, ensuring understanding and effectiveness.
  • Meet productivity requirements consistently, demonstrating clinical skills and techniques effectively.
  • Participate in discharge planning, addressing patient needs and recommending further interventions.
  • Ensure accurate scheduling and timely communication with healthcare teams and patients.

Job Requirements

  • Doctoral Degree in Physical Therapy is required; Bachelor’s Degree in Physical Therapy is acceptable.
  • Minimum of six months of Physical Therapy experience; three years preferred.
  • Physical Therapist License and AHA Basic Health Care Life Support HCP Certification are mandatory.
  • Proficiency in PC usage and documentation systems is essential.
  • Strong ability to develop and implement individualized treatment plans effectively.
  • Experience in supervising and coordinating with Physical Therapy Assistants is advantageous.
  • Knowledge of home healthcare policies and procedures is beneficial.
  • Excellent communication skills for patient education and team coordination.
  • Flexibility to adapt to various patient needs and organizational requirements.
