Key Responsibilities & Duties
- Conduct comprehensive patient evaluations and develop individualized therapy programs with measurable goals.
- Prepare and maintain accurate documentation, including progress notes, reassessments, and discharge summaries.
- Coordinate with physicians and healthcare teams to ensure effective resource utilization and care delivery.
- Supervise Physical Therapy Assistants, ensuring proper intervention execution and regulatory compliance.
- Educate patients and caregivers on therapy programs and provide comprehensive support.
- Implement treatment plans using appropriate clinical skills and techniques.
- Participate in discharge planning and address patient needs throughout care.
- Update and submit schedules accurately, responding to communications promptly.
- Adhere to organizational competencies and standards of behavior.
Job Requirements
- Doctoral degree in Physical Therapy (PT) preferred; Bachelor's degree required.
- Minimum of six months of physical therapy experience; three years preferred.
- Active Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
- Proficiency in PC usage and documentation systems.
- Ability to lift, push, and pull a minimum of 50 lbs. and stand for extended periods.
- Strong communication and coordination skills for effective patient and team interaction.
- Flexibility to adapt to varying job responsibilities and patient needs.
