Job Description

Overview

  • Provide professional rehabilitation therapy to patients in their homes, ensuring individualized care plans and goal-oriented therapy programs.
  • Work a hybrid schedule with flexibility, covering various cities within Monmouth County.

Key Responsibilities & Duties

  • Conduct comprehensive patient evaluations and develop individualized therapy programs with measurable goals.
  • Prepare and maintain accurate documentation, including progress notes, reassessments, and discharge summaries.
  • Coordinate with physicians and healthcare teams to ensure effective resource utilization and care delivery.
  • Supervise Physical Therapy Assistants, ensuring proper intervention execution and regulatory compliance.
  • Educate patients and caregivers on therapy programs and provide comprehensive support.
  • Implement treatment plans using appropriate clinical skills and techniques.
  • Participate in discharge planning and address patient needs throughout care.
  • Update and submit schedules accurately, responding to communications promptly.
  • Adhere to organizational competencies and standards of behavior.

Job Requirements

  • Doctoral degree in Physical Therapy (PT) preferred; Bachelor's degree required.
  • Minimum of six months of physical therapy experience; three years preferred.
  • Active Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
  • Proficiency in PC usage and documentation systems.
  • Ability to lift, push, and pull a minimum of 50 lbs. and stand for extended periods.
  • Strong communication and coordination skills for effective patient and team interaction.
  • Flexibility to adapt to varying job responsibilities and patient needs.
  • Commitment to delivering high-quality, patient-centered care.
  • Knowledge of therapy program development and implementation processes.
