Job Description

Overview

  • Provide comprehensive, patient-centered care in a dynamic healthcare environment, emphasizing health promotion and disease prevention.
  • Evaluate and manage acute and chronic conditions using evidence-based practices and collaborative approaches.
  • Perform holistic assessments, including mental health evaluations, to address diverse patient needs.
  • Order and interpret diagnostic tests to guide clinical decision-making and personalized treatment plans.
  • Prescribe medications and therapies aligned with clinical guidelines and patient-specific requirements.
  • Participate in quality improvement initiatives to enhance care delivery and patient outcomes.
  • Collaborate with multidisciplinary teams to ensure seamless and coordinated care.
  • Maintain accurate documentation in electronic medical records systems to support effective care management.
  • Engage in community health promotion activities to support organizational goals and outreach efforts.

Key Responsibilities & Duties

  • Deliver patient care, including evaluation, diagnosis, and management of diverse health conditions.
  • Educate patients on lifestyle changes, preventive measures, and treatment options to promote overall well-being.
  • Coordinate referrals to specialists and other healthcare providers to ensure comprehensive care.
  • Participate in on-call rotations to ensure continuous patient care availability.
  • Attend team meetings to foster collaboration and share insights for improved patient care.
  • Engage in pre-visit planning activities to optimize patient care delivery and efficiency.
  • Ensure compliance with healthcare regulations, including HIPAA and mandatory reporting requirements.
  • Pursue continuing education opportunities to maintain certifications and enhance clinical skills.
  • Provide culturally sensitive care to diverse populations, including immigrants and underserved communities.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and advanced nursing degree (MSN or DNP).
  • Certification as a Family Nurse Practitioner or Adult Nurse Practitioner.
  • Active NYS nursing license and DEA registration.
  • Current BLS and infection control certifications.
  • Proficiency in Spanish or an Asian language is preferred.
  • AAHIVM certification or LGBT health certification is advantageous.
  • Minimum of 2 years of experience as a Nurse Practitioner in a clinical setting.
  • Strong interpersonal skills and commitment to delivering compassionate, patient-focused care.
  • Ability to work effectively with diverse populations and multidisciplinary teams.
