Overview
- Provide direct mental health services and depression care management in a hybrid work environment.
- Collaborate with healthcare teams to deliver high-quality patient care and psychiatric consultations.
- Utilize standardized tools for patient assessment and behavioral health diagnosis management.
- Participate in clinical supervision and continuous quality improvement initiatives.
- Document patient care activities promptly and accurately in electronic health records.
- Develop linkages with community organizations for comprehensive patient support.
- Contribute to the design and implementation of clinical pathways for chronic condition treatment.
- Work with diverse populations, including HIV patients, homeless individuals, and the elderly.
Key Responsibilities & Duties
- Provide collaborative care services, including mental health services and telephonic outreach.
- Coordinate care for patients managed in primary care settings.
- Refer patients to long-term counseling and necessary services like substance abuse treatment.
- Utilize PHQ-9 and other tools for patient assessment following agency guidelines.
- Participate in department and organizational CQI projects as required.
- Manage caseload effectively using reporting tools for self-management.
- Engage in clinical supervision and interdisciplinary team collaboration.
- Design and implement protocols for selected chronic conditions treatment.
Job Requirements
- Master’s Degree in Social Work or related field required.
- NYS licensure as LMSW, LCSW, PhD, or PsyD required.
- Experience working with diverse populations preferred.
- Proficiency in electronic health records and MS Office applications required.
- Bilingual Spanish skills preferred with additional compensation.
- Knowledge of behavioral medicine and evidence-based treatments for depression required.
- Ability to make independent judgments in delivering high-standard social work care.
- Strong organizational, interpersonal, and communication skills required.
