Overview
- Provide direct depression care management and clinical services to patients in a hybrid work environment.
- Collaborate with healthcare teams to deliver high-quality mental health services and support.
- Utilize standardized tools for patient assessments and care planning.
- Engage in telephonic outreach and management of assigned cases effectively.
- Participate in clinical supervision and continuous quality improvement projects.

Key Responsibilities & Duties

Job Requirements
- Master’s Degree in Social Work or related field required.
- NYS licensure as LMSW, LCSW, PhD, or PsyD preferred.
- Experience working with diverse populations and healthcare settings preferred.
- Proficiency in electronic health records (EHR) and MS Office applications.
- Bilingual skills in Spanish preferred, with additional compensation available.
- Knowledge of behavioral medicine and evidence-based treatments for depression.
