Job Description

Overview

  • Provide high-quality nursing care to patients in a home healthcare setting, ensuring their well-being and recovery.
  • Educate patients and families on health prevention and healthy habits to promote long-term wellness.
  • Collaborate with healthcare professionals to develop and implement effective care plans tailored to patient needs.
  • Utilize current OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
  • Document patient care accurately and promptly using provided technology within required timeframes.
  • Adhere to health and safety guidelines and agency standards to ensure compliance with regulations.
  • Participate in training and orientation programs to maintain professional competency.
  • Support the agency's mission of providing culturally diverse and high-quality care.

Key Responsibilities & Duties

  • Conduct home health visits for assigned caseload, including admissions, re-certifications, and discharges.
  • Develop and implement physician care plans in collaboration with healthcare professionals.
  • Provide patient education on health prevention and wellness practices.
  • Ensure compliance with Department of Health and government regulations in all services.
  • Maintain accurate and timely documentation of patient care using agency-provided technology.
  • Utilize OASIS D1 assessments effectively for patient admissions and care transitions.
  • Collaborate with the Clinical Manager and other team members to ensure high-quality care delivery.
  • Participate in agency training programs to enhance professional skills and knowledge.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent qualification.
  • Valid Nursing license with proven experience as a registered nurse.
  • Experience in acute care or home healthcare settings preferred.
  • Proficiency in completing comprehensive OASIS assessments.
  • Familiarity with Homecare Homebase (HCHB) software is advantageous.
  • Ability to drive and access a personal vehicle for patient visits.
  • Strong communication skills to educate patients and families effectively.
  • Commitment to adhering to health and safety guidelines and agency standards.
