Overview

Key Responsibilities & Duties
- Conduct home health visits for assigned caseload, including admissions, re-certifications, and discharges.
- Develop and implement physician care plans in collaboration with healthcare professionals.
- Provide patient education on health prevention and wellness practices.
- Ensure compliance with Department of Health and government regulations in all services.
- Maintain accurate and timely documentation of patient care using agency-provided technology.
- Utilize OASIS D1 assessments effectively for patient admissions and care transitions.
- Collaborate with the Clinical Manager and other team members to ensure high-quality care delivery.
- Participate in agency training programs to enhance professional skills and knowledge.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Valid Nursing license with proven experience as a registered nurse.
- Experience in acute care or home healthcare settings preferred.
- Proficiency in completing comprehensive OASIS assessments.
- Familiarity with Homecare Homebase (HCHB) software is advantageous.
- Ability to drive and access a personal vehicle for patient visits.
- Strong communication skills to educate patients and families effectively.
- Commitment to adhering to health and safety guidelines and agency standards.
