Job Description

Overview

  • Provide skilled nursing services within a certified home health agency, ensuring safe and effective care in patients' homes.
  • Collaborate with patients, families, and physicians to develop and implement personalized care plans.
  • Conduct clinical assessments to evaluate patient needs and home care safety.
  • Monitor and adjust care plans as necessary to meet evolving patient requirements.
  • Provide oversight and guidance to Home Health Aide services for optimal patient support.
  • Participate in interdisciplinary care delivery processes to ensure comprehensive patient care.
  • Utilize bilingual skills to enhance communication and care delivery, if applicable.
  • Engage in professional development opportunities to maintain and enhance nursing expertise.

Key Responsibilities & Duties

  • Conduct initial and ongoing patient assessments to determine home care needs and safety.
  • Develop, oversee, and implement individualized care plans in collaboration with interdisciplinary teams.
  • Supervise Home Health Aides to ensure adherence to care plans and patient safety.
  • Monitor patient progress and adjust care plans as necessary to address changing conditions.
  • Perform Patient Review Instrument (PRI) assessments to evaluate patient needs.
  • Maintain accurate documentation of patient care activities and outcomes.
  • Ensure compliance with all regulatory and organizational standards in care delivery.
  • Provide education and support to patients and families regarding care plans and health management.

Job Requirements

  • Current New York State RN license and registration required.
  • Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
  • Minimum of 2 years of experience in home care assessments and care plan development.
  • At least 1 year of supervisory experience in a hospital or home care setting.
  • Proficiency in Patient Review Instrument (PRI) assessments preferred.
  • Valid driver’s license and ability to travel within the Manhattan area.
  • Proficiency in Microsoft Office applications for documentation and communication.
  • Bilingual skills are advantageous for effective patient and family communication.
