Overview
- Provide specialized medical care to hospice and palliative patients, ensuring compliance with regulations and policies.
- Collaborate with interdisciplinary teams to develop and revise patient care plans.
- Offer medical expertise in terminal prognosis evaluation and symptom management.
- Conduct home visits and provide patient and family support as needed.
- Review medication regimens for appropriateness and adverse reactions.
- Facilitate seamless transitions from home care to inpatient care.
- Ensure hospice eligibility and provide leadership to enhance hospice utilization.
- Participate in education and innovation to improve hospice services.
Key Responsibilities & Duties
- Evaluate and recertify terminal prognosis at regular intervals.
- Collaborate with attending physicians and referral sources for patient care.
- Provide medical care within hospice regulations and guidelines.
- Document all patient care services in electronic medical records.
- Lead interdisciplinary teams with professionalism and expertise.
- Support patients and families through sensitive medical conversations.
- Review and determine hospice eligibility for referred patients.
- Provide medical expertise for pre-admission symptom management.
- Enhance hospice utilization through education and collaboration.
Job Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Hospice and Palliative Medicine or related specialty.
- Current and unrestricted medical license in New York State.
- DEA license and prescriptive authority as per regulations.
- Minimum of 2-3 years of experience in hospice or palliative care preferred.
- Proficiency in electronic medical records and documentation standards.
- Valid driver’s license and reliable transportation for community visits.
- Current BLS/ACLS certification preferred.
