Job Description

Overview

  • Provide essential hospice care during after-hours shifts, addressing urgent physical and psycho-social issues remotely and in the field.
  • Coordinate with the Call Center Triage and On-Call Manager to ensure patient needs are met effectively and promptly.
  • Implement care plans, educate patients and caregivers on medication management, safety, and symptom management.
  • Attend patient deaths to offer emotional support and guidance to families.
  • Communicate with the hospice team to report on patient visits, care plan changes, and follow-up needs.
  • Adhere to New York State Standards for Professional Registered Nurses in all aspects of care delivery.
  • Work in a hybrid environment, combining remote and field-based responsibilities.

Key Responsibilities & Duties

  • Respond to triage visit requests via cell phone, coordinating with the on-call team and hospice physician.
  • Provide unscheduled patient visits for urgent needs during assigned shifts.
  • Educate patients and caregivers on care plan adjustments, including medication and symptom management.
  • Attend patient deaths to offer emotional support and guidance to families.
  • Report patient visit outcomes and care plan updates to the triage and primary hospice team.
  • Utilize Uber transportation for field visits across boroughs as required.
  • Maintain compliance with NYS Standards for Professional Registered Nurses.

Job Requirements

  • Associate of Applied Science (AAS) degree from an accredited School of Nursing.
  • Minimum of 2 years of experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing.
  • New York State RN licensure and valid NYS driver’s license preferred.
  • Proficiency in Word, Excel, and Windows applications.
  • Commitment to sit for the national board certification CHPN within one year of hire.
  • Ability to work hybrid shifts, including weekends and after-hours.
