Overview
- Provide medical case management to clients in permanent supportive housing, addressing complex medical and psychiatric needs.
- Collaborate with healthcare professionals to optimize client outcomes and ensure continuity of care.
- Perform health and vital checks, documenting findings and maintaining medication records.
- Offer health education and advocate for client health preferences and goals.
- Assist clients with practical needs, such as navigating services and resources.
- Work on-site during specified hours, including weekends, ensuring consistent client support.
- Provide emergency first aid and CPR assistance as needed.
- Participate in treatment team meetings and case conferences to share insights and coordinate care.
Key Responsibilities & Duties
- Supervise assigned staff and ensure adherence to medical and psychosocial protocols.
- Link clients to appropriate medical and psychiatric services and coordinate with healthcare providers.
- Act as primary medical case manager, managing health needs and conducting vital checks.
- Document client interactions, medication administration records, and controlled substance logs.
- Provide health education and advocate for client health preferences and goals.
- Assist clients with practical needs, such as accessing resources and understanding bills.
- Participate in team meetings and case conferences to ensure coordinated care.
- Provide emergency first aid and CPR assistance when required.
- Perform related duties as assigned to support client health and well-being.
Job Requirements
- Valid Licensed Practical Nurse (LPN) certification is required.
- Associate of Science (AS) degree in nursing or a related field.
- Minimum of 1 year of experience, with 3 years preferred.
- Strong knowledge of medical case management and psychiatric rehabilitation.
- Proficiency in documenting client interactions and maintaining medication records.
- Ability to provide health education and advocate for client health goals.
- Experience with emergency first aid and CPR procedures.
- Excellent communication and organizational skills.
- Availability to work specified hours, including weekends.
