Overview
- Coordinate hospice care services for patients across multiple hospital locations.
- Assess patients for hospice eligibility and facilitate admissions in compliance with regulations.
- Develop and maintain strong relationships with hospital partners to support business goals.
- Provide specialized clinical care to hospice inpatients with compassion and expertise.
- Collaborate with interdisciplinary teams to create individualized care plans.
- Educate patients, families, and referrers about hospice services and benefits.
- Ensure accurate and detailed documentation of patient care and services.
- Work flexible hours, including weekends, to meet patient and organizational needs.
Key Responsibilities & Duties
- Conduct comprehensive patient assessments for hospice or palliative care eligibility.
- Collaborate with hospital personnel to facilitate hospice admissions.
- Coordinate daily hospice care for patients at the General Inpatient level.
- Develop and strengthen business relationships with hospital partners.
- Respond to referral requests and provide necessary information to hospital staff.
- Utilize regulatory guidelines to determine hospice eligibility.
- Participate as an RN member of the interdisciplinary hospice team.
- Educate patients, families, and healthcare providers about hospice services.
Job Requirements
- Associate of Applied Science (AAS) in nursing or related field.
- Minimum of two years of experience in medical-surgical or community health nursing.
- Preferred experience in hospice or oncology nursing.
- New York State RN licensure and CPR-BLS certification required.
- Valid NYS Driver's license preferred.
- Proficiency in Microsoft Office applications.
- Strong communication, organizational, and critical thinking skills.
- Ability to work non-judgmentally with diverse patient populations.
