Overview
- Provide compassionate and patient-centered care to individuals in hospice settings, ensuring their physical, emotional, and psychosocial needs are met.
- Develop and implement individualized nursing care plans tailored to the unique needs of hospice patients.
- Collaborate with interdisciplinary teams to deliver holistic and comprehensive hospice care services.
- Supervise and support home health aides and personal care workers in providing quality patient care.
- Request and coordinate physician orders to ensure optimal adjustments to patient care plans.
- Maintain accurate and detailed documentation of clinical data and patient progress.
- Educate patients and families about hospice care processes, expectations, and available resources.
- Ensure compliance with healthcare regulations and standards in hospice care practices.
Key Responsibilities & Duties
- Conduct comprehensive physical and psychosocial assessments for hospice patients to determine their care needs.
- Design and implement nursing care plans that address individual patient requirements and preferences.
- Collaborate with physicians and other healthcare professionals to modify care plans as necessary.
- Supervise, train, and evaluate home health aides and personal care workers to ensure quality care delivery.
- Provide education and emotional support to patients and families during challenging end-of-life stages.
- Maintain accurate and timely documentation of patient care activities and outcomes.
- Participate in interdisciplinary team meetings to coordinate and optimize patient care strategies.
- Adhere to established hospice care standards, protocols, and regulatory requirements.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent educational qualification required.
- Registered Nurse (RN) licensure with a minimum of 2 years of clinical experience.
- Experience in hospice, home care, or acute care settings is highly preferred.
- Strong communication, teaching, and interpersonal skills to effectively interact with patients and families.
- Ability to work collaboratively within an interdisciplinary healthcare team environment.
- Proficiency in clinical data documentation and management systems.
- Commitment to delivering compassionate and empathetic care to patients in hospice settings.
- Knowledge of hospice care standards, protocols, and regulations is advantageous.
