Overview
- Provide medical social services or nursing care to senior patients in collaboration with healthcare teams.
- Deliver home-based medical social services under the supervision of the Administrator and Director of Nursing.
- Perform socio-psychological evaluations and environmental assessments to optimize patient care.
- Assist patients and families in understanding and following medical recommendations.
- Coordinate community services and resources for safe and effective home care.
- Maintain accurate and complete medical records and documentation.
- Communicate effectively with patients, families, and healthcare professionals to ensure optimal care.
- Participate in quality assurance processes to improve service delivery.
- Travel to patient residences to provide necessary care and assessments.
Key Responsibilities & Duties
- Receive and process intake referrals, preparing necessary documentation and tools.
- Conduct home safety checks and environmental assessments for patient care planning.
- Perform socio-psychological evaluations and document emergency contacts and support systems.
- Provide counseling and casework to restore patients' social adjustment levels.
- Act as an intermediary with community resources and health agencies for patient support.
- Communicate effectively with healthcare teams and physicians to optimize patient treatment.
- Self-schedule caseloads while maintaining communication with supervisors.
- Submit accurate weekly documentation to ensure up-to-date medical records.
- Participate in case conferencing and quality assurance committees as required.
Job Requirements
- Associate of Applied Science (AAS) degree required; MSW preferred for social worker role.
- Minimum of 1 year of experience; 3 years preferred in senior care settings.
- Licensed Practical Nurse (LPN) or Medical Social Worker (MSW) certification required.
- Proficiency in using computers and office equipment for documentation and communication.
- Ability to travel to patient residences for care delivery.
- Strong communication skills in English for effective patient and team interactions.
- Knowledge of community resources and ability to coordinate patient services.
- Experience in nursing homes, assisted living, or hospital settings preferred.
