Overview
- Serve as an Enrollment Registered Nurse, assisting members with enrollment processes and service planning.
- Conduct assessments and develop person-centered service plans tailored to individual needs.
- Collaborate with members to determine service levels and hours based on NYIA reviews.
- Work primarily remotely with occasional field visits as required.
- Participate in initial in-office training sessions to ensure role readiness.
- Maintain flexibility to meet with members when requested, ensuring high-quality service delivery.

Key Responsibilities & Duties
- Conduct member assessments using UAS tools to evaluate service needs.
- Develop and review person-centered service plans with members prior to enrollment.
- Coordinate enrollment processes, ensuring compliance with organizational standards.
- Provide guidance and support to members during the enrollment phase.
- Participate in training sessions to enhance knowledge and skills.
- Collaborate with colleagues to optimize enrollment procedures and outcomes.
- Maintain accurate records and documentation of member interactions and plans.

Job Requirements
- Bachelor of Science (BS) degree in Nursing required.
- Registered Nurse (RN) license in the State of New York mandatory.
- Minimum of 1 year of relevant experience in nursing or enrollment roles.
- Proficiency in UAS tools and processes preferred.
- Ability to work in a hybrid environment with occasional field visits.
