Job Description

Overview

  • Lead the development and operations of a Children's Health Home program under Medicaid guidelines.
  • Build infrastructure, workflows, and teams for a newly established initiative.
  • Ensure compliance with NYC and NYS Medicaid Children’s Health Home requirements.
  • Adapt proven systems from an established Adult Health Home program.
  • Provide operational leadership and translate policy into practice effectively.
  • Oversee program performance, quality, and growth initiatives.
  • Collaborate with stakeholders to ensure effective cross-system coordination.
  • Support children and families with complex medical, behavioral health, or social needs.

Key Responsibilities & Duties

  • Design and implement Children’s Health Home workflows, policies, and procedures.
  • Ensure program alignment with NYS DOH Children’s Health Home standards.
  • Oversee daily program operations, including enrollment and service delivery.
  • Monitor caseloads, service utilization, and outcomes for compliance.
  • Implement quality management systems and corrective action plans.
  • Recruit, supervise, and mentor care managers and program staff.
  • Establish training and performance expectations for program staff.
  • Build partnerships with networks, providers, schools, and community organizations.
  • Represent the client in Health Home meetings and external forums.

Job Requirements

  • Master’s degree in Social Work, Public Health, or related field required.
  • Clinical licensure preferred (LMSW, LCSW, LMH, or equivalent).
  • Minimum 3 years of program management experience serving children and families.
  • At least 2 years of supervisory experience within Medicaid Health Home or similar model.
  • Strong familiarity with Medicaid systems and Children’s Health Home models.
  • Experience with program launch, operations, and regulatory compliance.
  • Proficiency in EHR platforms and operational leadership skills.
  • Bilingual Spanish skills are a plus.
  • Comfortable leading in a start-up or growth environment.
