Job Description

Overview

  • Lead the establishment and management of a Children's Health Home program, ensuring compliance with Medicaid requirements and delivering high-quality care coordination services.
  • Design and implement workflows, policies, and procedures tailored to children and families, adapting best practices from existing adult programs.
  • Collaborate with leadership to achieve program growth, readiness milestones, and long-term sustainability goals.
  • Oversee daily operations, including enrollment, care assignment, and service delivery, ensuring benchmarks are met.
  • Ensure compliance with Medicaid regulations and implement quality management systems to monitor program performance.
  • Recruit, supervise, and mentor program staff, fostering a culture of accountability and continuous improvement.
  • Develop partnerships with providers and community organizations to promote effective cross-system coordination for complex needs.
  • Represent the organization in external forums, advocating for children and families served by the program.

Key Responsibilities & Duties

  • Lead the design and implementation of workflows, policies, and procedures for the Children's Health Home program.
  • Ensure program structure aligns with Medicaid standards, including eligibility, consent, assessments, and care planning.
  • Monitor caseloads, service utilization, and outcomes to meet contractual and regulatory benchmarks.
  • Implement quality management systems, including chart reviews, audits, and corrective action plans.
  • Recruit, supervise, and mentor care managers and program staff, providing clinical and operational consultation.
  • Establish training, onboarding, and performance expectations aligned with program requirements.
  • Build partnerships with Health Home networks, providers, schools, and community-based organizations.
  • Represent the organization in Health Home meetings and external forums to advocate for program goals.

Job Requirements

  • Master's degree in Social Work, Public Health, Public Administration, or related field; clinical licensure preferred.
  • Minimum 3 years of program management or care coordination experience serving children and families with complex needs.
  • At least 2 years of supervisory experience, preferably within a Medicaid Health Home or similar care management model.
  • Strong familiarity with Medicaid systems, Children's Health Home models, and EHR platforms.
  • Ability to translate policy and regulation into practice, ensuring compliance and operational excellence.
  • Demonstrated experience with program launch, operations, and regulatory compliance.
  • Bilingual Spanish skills are a plus but not required.
  • Comfort leading in a start-up or growth environment, fostering collaboration and continuous improvement.
