Job Description

Overview

  • Lead the establishment of a Children's Health Home program under Medicaid guidelines.
  • Develop workflows, policies, and procedures tailored for children and families.
  • Adapt existing adult program models to meet pediatric-specific needs.
  • Ensure compliance with NYC and NYS Medicaid regulations and standards.
  • Oversee program operations, quality assurance, and regulatory adherence.
  • Recruit, train, and supervise program staff for effective service delivery.
  • Collaborate with stakeholders and community organizations to enhance care coordination.
  • Monitor program performance and implement improvements for scalability.
  • Provide leadership in a dynamic and growth-focused environment.

Key Responsibilities & Duties

  • Design and implement workflows, policies, and procedures for the program.
  • Ensure program compliance with Medicaid Children’s Health Home standards.
  • Oversee daily operations, including enrollment and care coordination.
  • Monitor service delivery and outcomes to meet benchmarks.
  • Recruit, supervise, and mentor program staff for effective service delivery.
  • Implement quality management systems and corrective action plans.
  • Collaborate with stakeholders to enhance cross-system coordination.
  • Represent the program in external meetings and forums.
  • Lead program growth and sustainability efforts in partnership with leadership.

Job Requirements

  • Master’s degree in Social Work, Public Health, or related field.
  • Clinical licensure preferred (LMSW, LCSW, LMH, or equivalent).
  • Minimum 3 years of program management experience in care coordination.
  • At least 2 years of supervisory experience, preferably in Medicaid programs.
  • Strong familiarity with Medicaid systems and Children’s Health Home models.
  • Experience with program launch and regulatory compliance.
  • Proficiency in EHR platforms and operational leadership.
  • Bilingual Spanish skills are a plus.
  • Comfortable working in a start-up or growth-focused environment.
