Job Description

Overview

  • Provide comprehensive nursing care to clients in a supportive environment.
  • Collaborate with multidisciplinary teams to optimize client health outcomes.
  • Maintain accurate medical records and medication administration logs.
  • Educate clients on health practices and advocate for their health goals.
  • Conduct health and vitals checks for assigned clients.
  • Coordinate medical treatment referrals and communicate with healthcare providers.
  • Provide emergency first aid and CPR assistance when needed.
  • Participate in treatment team meetings and case conferences.

Key Responsibilities & Duties

  • Supervise assigned staff and ensure compliance with medical protocols.
  • Act as primary medical case manager for assigned clients.
  • Coordinate information with treating physicians, pharmacies, and other treatment collaterals.
  • Document findings and maintain onsite medication administration records.
  • Ensure adequate medication supply through med counts and follow-ups.
  • Provide health education directly to clients and support their health preferences.
  • Assist clients in developing health goals in collaboration with team members.
  • Perform related duties as assigned to support client care.

Job Requirements

  • Licensed Practical Nurse (LPN) certification is required.
  • Associate of Science (AS) degree in nursing or related field.
  • Minimum of 1 year of nursing experience; 3 years preferred.
  • Proficiency in medical case management and client health advocacy.
  • Strong documentation skills for maintaining accurate medical records.
  • Ability to provide emergency first aid and CPR assistance.
  • Excellent communication skills for coordinating with healthcare providers.
