Overview

• Lead and oversee home care nursing operations, ensuring quality patient care and compliance with healthcare regulations.
• Coordinate and evaluate individualized care plans, collaborating with families and professional caregivers.
• Provide mentorship and supervision to nursing staff during field visits and daily operations.
• Utilize critical thinking and organizational skills to address complex patient care scenarios.
• Ensure accurate documentation and communication of patient information to relevant stakeholders.

Key Responsibilities & Duties

• Develop and implement comprehensive care plans tailored to individual patient needs.
• Supervise and mentor nursing staff, ensuring adherence to best practices and protocols.
• Conduct field visits to assess patient conditions and provide direct support to nurses.
• Collaborate with interdisciplinary teams to optimize patient outcomes and resource utilization.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Ensure compliance with state and federal home health regulations and standards.
• Coordinate insurance reviews and manage documentation for reimbursement processes.
• Provide training and orientation for new nursing staff, fostering professional growth.

Job Requirements

• Associate of Applied Science (AAS) degree in Nursing; BSN preferred.
• Minimum of 6 years of nursing experience, with 3 years in home care.
• NYS RN license and valid driver’s license required; Oasis certification preferred.
• Proven ability to supervise, mentor, and coach nursing staff effectively.
• Strong clinical skills and knowledge of home health care regulations.
• Exceptional organizational and multitasking abilities to prioritize tasks efficiently.
• Proficiency in computer systems and documentation software.
• Excellent communication and problem-solving skills within interdisciplinary teams.
