Job Description

Overview

  • Support the Pharmacy Quality program by implementing initiatives to meet corporate STAR goals and improve medication adherence.
  • Conduct comprehensive medication reviews and develop care plans to address barriers and optimize patient outcomes.
  • Collaborate with interdisciplinary teams to enhance medication therapy management and ensure compliance with CMS standards.
  • Provide patient education on disease states, medication actions, side effects, and compliance requirements.
  • Document interactions and care plans using applicable software systems for accurate record-keeping.
  • Assist in managing medication costs through cost-effective interventions and formulary assessments.
  • Evaluate and maintain formulary setup based on clinical requirements and regulatory compliance.
  • Participate in projects and duties as assigned to support pharmacy operations and quality improvement.

Key Responsibilities & Duties

  • Conduct patient medication history reviews and identify barriers to care for resolution.
  • Recommend changes in drug therapy and lifestyle based on patient interviews and medical records.
  • Resolve and prevent medication-related problems, including adverse drug reactions and drug interactions.
  • Monitor patient outcomes and compliance to optimize therapy and minimize unnecessary drug use.
  • Deliver medication therapy management services and document according to CMS standards.
  • Coordinate medication reconciliation to prevent readmissions and adverse drug reactions.
  • Collaborate with prescribers and interdisciplinary teams to develop patient-specific therapy goals.
  • Ensure compliance with clinical standards and maintain up-to-date clinical knowledge.

Job Requirements

  • Bachelor’s Degree in Pharmacy or PharmD from an accredited institution.
  • Current, unrestricted Pharmacist License as per state regulations.
  • Completion of MTM training program by APhA or PowerPak C.E. preferred.
  • PGY-1 residency in community pharmacy, ambulatory care, or managed care pharmacy preferred.
  • Clinical Pharmacy Board Certification via BPS or NBMTM preferred.
  • 10+ years of relevant professional experience with 3–5 years in a related role.
  • Strong knowledge of CMS Comprehensive Medication Reviews and managed care practices.
  • Proficiency in clinical therapeutic skills and medication dosing for geriatric and adult populations.
