Overview
- Support the Pharmacy Quality program to achieve corporate STAR goals and improve medication adherence through comprehensive care plans.
- Conduct medication reviews and develop strategies to optimize therapy and resolve barriers for patients.
- Collaborate with interdisciplinary teams to enhance patient outcomes and ensure regulatory compliance.
- Provide patient education on disease states, medication actions, and adherence strategies.
- Document interactions and care plans using applicable software systems.
- Assist with medication reconciliation to prevent readmissions and adverse drug reactions.
- Evaluate and maintain formulary setup based on clinical requirements and compliance standards.
- Participate in projects and duties as assigned to support pharmacy initiatives.
Key Responsibilities & Duties
- Conduct patient medication history reviews and identify barriers to care, collaborating with patients to resolve them.
- Recommend changes in drug therapy and lifestyle based on patient interviews and monitoring parameters.
- Identify, resolve, and prevent medication-related problems such as adverse reactions and drug interactions.
- Provide patient monitoring to optimize outcomes and ensure compliance with therapy plans.
- Deliver Medication Therapy Management services to assess and optimize medication therapy.
- Coordinate medication reconciliation across care continuum to address adherence concerns and monitor outcomes.
- Manage medication costs through cost-effective interventions and formulary assessments.
- Collaborate with prescribers and interdisciplinary teams to develop patient-specific therapy goals.
- Ensure compliance with clinical standards and regulatory requirements for pharmacy services.
Job Requirements
- Bachelor’s Degree in Pharmacy or PharmD from an accredited college of pharmacy.
- Current, unrestricted Pharmacist License in accordance with state licensing requirements.
- Completion of MTM training program by APhA or PowerPak C.E. preferred.
- PGY-1 residency program in community pharmacy or managed care pharmacy preferred.
- Clinical Pharmacy Board Certification via BPS or NBMTM preferred.
- Minimum of 3 years of relevant experience, with 5 years preferred.
- Strong knowledge of CMS Comprehensive Medication Reviews and managed care practices.
- Proficiency in clinical therapeutic skills and STAR measures improvement methods.
- Excellent communication, critical thinking, and organizational skills.
