Job Description

Overview

  • Serve as an On Call Hospice Triage Supervisor, providing leadership and guidance to call center staff during rotating shifts.
  • Ensure quality telephonic intervention for patients based on their Plan of Care, addressing clinical and non-clinical issues.
  • Collaborate with Team Managers, AOC, and Hospice MDs to resolve complex patient care situations.
  • Work closely with On Call RNs to provide field support when necessary, ensuring seamless patient care.
  • Maintain compliance with organizational policies and procedures, promoting best practices in hospice care.
  • Contribute to the development of staff by providing mentorship and fostering a collaborative work environment.
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
  • Uphold the mission and values of the client organization, ensuring compassionate care for patients and families.

Key Responsibilities & Duties

  • Provide leadership and guidance to call center staff during rotating shifts, ensuring effective communication and problem resolution.
  • Address clinical and non-clinical issues promptly, escalating complex situations to appropriate personnel.
  • Deliver professional telephonic interventions to patients, adhering to established care plans.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and support.
  • Monitor and evaluate staff performance, providing constructive feedback and support for professional growth.
  • Ensure compliance with regulatory standards and organizational policies in all patient care activities.
  • Participate in staff training programs to enhance skills and knowledge in hospice care practices.
  • Maintain accurate documentation of patient interactions and care provided during on-call shifts.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution; BS degree preferred.
  • Minimum of 4 years of hospice nursing experience; supervisory experience preferred.
  • Current New York State RN licensure; valid NYS license preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • Strong leadership and communication skills, with the ability to manage rotating shifts effectively.
  • Proficiency in telephonic patient care and intervention techniques.
  • Commitment to providing compassionate and high-quality hospice care.
