Job Description

Overview

  • Provide high-quality nursing care to patients in their homes, ensuring their well-being and speedy recovery.
  • Educate patients and families on health prevention and healthy habits to promote long-term wellness.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Utilize advanced nursing skills to assess, plan, and deliver care with minimal supervision.
  • Ensure compliance with health and safety guidelines and regulatory standards.
  • Maintain accurate and timely documentation using provided tools and systems.
  • Participate in ongoing training and professional development activities.
  • Support the agency's mission by adhering to standards of care and communication.

Key Responsibilities & Duties

  • Conduct home health visits for assigned caseloads, including admissions, re-certifications, and discharges.
  • Collaborate with physicians and healthcare teams to develop comprehensive care plans.
  • Perform OASIS D1 assessments for patient admissions, transfers, and discharges.
  • Ensure all services comply with health department and government regulations.
  • Provide patient education on health maintenance and disease prevention.
  • Document care activities accurately within the required timeframe using provided tools.
  • Maintain effective communication with patients, families, and healthcare professionals.
  • Participate in agency training programs and adhere to cultural diversity standards.

Job Requirements

  • Valid Nursing license and proven experience as a Registered Nurse.
  • Associate of Applied Science (AAS) in Nursing or equivalent qualification.
  • Experience in acute care, hospital settings, or home care environments.
  • Proficiency in completing comprehensive OASIS assessments.
  • Familiarity with Homecare Homebase (HCHB) is advantageous.
  • Ability to drive and access to a reliable vehicle.
  • Strong communication and organizational skills.
