Key Responsibilities & Duties
- Conduct home health visits for assigned caseloads, including admissions, re-certifications, and discharges.
- Collaborate with physicians and healthcare teams to develop comprehensive care plans.
- Perform OASIS D1 assessments for patient admissions, transfers, and discharges.
- Ensure all services comply with health department and government regulations.
- Provide patient education on health maintenance and disease prevention.
- Document care activities accurately within the required timeframe using provided tools.
- Maintain effective communication with patients, families, and healthcare professionals.
- Participate in agency training programs and adhere to cultural diversity standards.
Job Requirements
- Valid Nursing license and proven experience as a Registered Nurse.
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Experience in acute care, hospital settings, or home care environments.
- Proficiency in completing comprehensive OASIS assessments.
- Familiarity with Homecare Homebase (HCHB) is advantageous.
- Ability to drive and access to a reliable vehicle.
- Strong communication and organizational skills.
