Key Responsibilities & Duties
- Conduct home visits for patient care, including admissions, re-certifications, and discharges.
- Develop and execute care plans in coordination with physicians and healthcare teams.
- Educate patients and families on preventive health measures and wellness strategies.
- Ensure compliance with healthcare regulations and organizational standards.
- Document patient interactions and care plans using agency-provided software.
- Utilize OASIS D1 assessments for patient care transitions and evaluations.
- Collaborate with the Clinical Manager to ensure optimal care delivery.
- Participate in training programs to stay updated on best practices in home healthcare.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Active Nursing license with experience as a registered nurse.
- Preferred experience in acute care or home healthcare settings.
- Proficiency in conducting OASIS D1 assessments.
- Familiarity with Homecare Homebase (HCHB) software is advantageous.
- Ability to drive and access a personal vehicle for patient visits.
- Strong communication skills for patient and family education.
- Commitment to maintaining health and safety standards in care delivery.
