Field Nurse

Overview

• Deliver exceptional nursing care to patients in home healthcare, ensuring their recovery and long-term wellness.
• Educate patients and families on health practices to promote sustainable well-being.

• Perform comprehensive OASIS D1 assessments for admissions, re-certifications, and discharges.

Key Responsibilities & Duties

• Conduct home visits for patient care, including admissions, re-certifications, and discharges.
• Develop and execute care plans in coordination with physicians and healthcare teams.
• Educate patients and families on preventive health measures and wellness strategies.
• Ensure compliance with healthcare regulations and organizational standards.
• Document patient interactions and care plans using agency-provided software.
• Utilize OASIS D1 assessments for patient care transitions and evaluations.
• Collaborate with the Clinical Manager to ensure optimal care delivery.
• Participate in training programs to stay updated on best practices in home healthcare.

Job Requirements

• Associate of Applied Science (AAS) in Nursing or equivalent qualification.
• Active Nursing license with experience as a registered nurse.
• Preferred experience in acute care or home healthcare settings.
• Proficiency in conducting OASIS D1 assessments.
• Familiarity with Homecare Homebase (HCHB) software is advantageous.
• Ability to drive and access a personal vehicle for patient visits.
• Strong communication skills for patient and family education.
• Commitment to maintaining health and safety standards in care delivery.
