Primary Care Physician

Key Responsibilities & Duties

• Diagnose and treat acute and chronic conditions, ensuring optimal patient outcomes.
• Develop and implement individualized care plans in collaboration with patients and care teams.
• Provide education and guidance to patients on preventive care and lifestyle modifications.
• Utilize electronic health records to document patient interactions and care plans efficiently.
• Lead and coordinate the efforts of the care team to ensure seamless service delivery.
• Engage in regular communication with specialists and hospitalists for coordinated care.
• Participate in quality improvement initiatives and adhere to clinical guidelines.

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
• Active state medical license and DEA certification for controlled substances.
• Minimum of 1 year of experience in primary care; 5 years preferred.
• Proficiency in electronic health records and comfortable with technology.
• Strong commitment to patient-centered care and value-based healthcare principles.
• Excellent communication and leadership skills to guide care teams effectively.
• Basic Life Support (BLS) certification required within 90 days of employment.
