Job Description

Overview

  • Provide specialized wound care services for patients in skilled nursing facilities.
  • Collaborate with interdisciplinary teams to enhance patient outcomes and care plans.
  • Prescribe medications and treatments adhering to state regulations and best practices.
  • Utilize advanced wound care products and techniques to manage complex cases.
  • Diagnose and treat various wound types, including diabetic and arterial wounds.
  • Ensure compliance with clinical guidelines and patient safety standards.
  • Maintain accurate documentation using electronic medical records systems.
  • Support long-term care facilities in improving CMS ratings through quality care.

Key Responsibilities & Duties

  • Provide wound care and primary services for long-term care patients.
  • Prescribe medications, including controlled substances, as per state laws.
  • Order and review diagnostic tests and treatments for patient care.
  • Collaborate with primary care physicians and nursing staff on patient care plans.
  • Diagnose and treat various types of wounds, including pressure injuries.
  • Utilize advanced wound care products and devices effectively.
  • Educate patients and staff on wound care best practices and prevention.
  • Participate in interdisciplinary team meetings to coordinate care strategies.

Job Requirements

  • Doctorate of Nursing Practice (DNP) with unrestricted NP license in Ohio.
  • Board certification and optional DEA licensure preferred.
  • Experience in adult, family, or geriatric specialties with wound care expertise.
  • Proficiency in diagnosing and treating various wound types and pressure injuries.
  • Knowledge of advanced wound care products and devices, including wound vacs.
  • Comfortable working in long-term care settings and using EMR systems.
  • Strong preference for wound certification or willingness to obtain certification.
  • Ability to collaborate within interdisciplinary teams to ensure optimal patient care.
