Overview
- Provide comprehensive nursing care and case management services for hospice patients in a hybrid work environment.
- Collaborate with interdisciplinary teams to ensure optimal patient care and support.
- Utilize community resources to plan and direct services for patients and families.
- Conduct initial assessments and develop individualized care plans based on nursing diagnoses.
- Administer prescribed medications and treatments while adhering to nursing standards.
- Educate patients and families on health care practices and discharge planning.
- Document patient care activities and communicate updates to physicians as required.
- Participate in staff meetings and contribute to team development initiatives.
Key Responsibilities & Duties
- Perform initial and ongoing assessments of patients to determine care needs.
- Develop and implement personalized care plans in collaboration with patients and families.
- Coordinate with physicians and interdisciplinary teams to ensure effective care delivery.
- Administer medications, treatments, and therapeutic procedures as prescribed.
- Provide health education and counseling to patients and their families.
- Document patient care activities accurately and update clinical notes regularly.
- Supervise home health aides and ensure adherence to care plans.
- Participate in case management meetings and share insights for improving care outcomes.
Job Requirements
- Bachelor’s degree in nursing from an accredited institution preferred.
- Current RN licensure and CPR certification required.
- Minimum of 1 year acute care or home care nursing experience; 5 years preferred.
- Proficiency in nursing skills, problem-solving, and communication abilities.
- Ability to lift and transfer patients and equipment as needed.
- Valid driver’s license and ability to travel in various weather conditions.
- Knowledge of computer systems is beneficial but not mandatory.
