Job Description

Overview

  • Provide comprehensive nursing care and case management services for hospice patients in a hybrid work environment.
  • Collaborate with interdisciplinary teams to ensure optimal patient care and support.
  • Utilize community resources to plan and direct services for patients and families.
  • Conduct initial assessments and develop individualized care plans based on nursing diagnoses.
  • Administer prescribed medications and treatments while adhering to nursing standards.
  • Educate patients and families on health care practices and discharge planning.
  • Document patient care activities and communicate updates to physicians as required.
  • Participate in staff meetings and contribute to team development initiatives.

Key Responsibilities & Duties

  • Perform initial and ongoing assessments of patients to determine care needs.
  • Develop and implement personalized care plans in collaboration with patients and families.
  • Coordinate with physicians and interdisciplinary teams to ensure effective care delivery.
  • Administer medications, treatments, and therapeutic procedures as prescribed.
  • Provide health education and counseling to patients and their families.
  • Document patient care activities accurately and update clinical notes regularly.
  • Supervise home health aides and ensure adherence to care plans.
  • Participate in case management meetings and share insights for improving care outcomes.

Job Requirements

  • Bachelor’s degree in nursing from an accredited institution preferred.
  • Current RN licensure and CPR certification required.
  • Minimum of 1 year acute care or home care nursing experience; 5 years preferred.
  • Proficiency in nursing skills, problem-solving, and communication abilities.
  • Ability to lift and transfer patients and equipment as needed.
  • Valid driver’s license and ability to travel in various weather conditions.
  • Knowledge of computer systems is beneficial but not mandatory.
