Overview
- Provide high-quality healthcare services as a Nurse Practitioner in a dynamic clinical setting.
- Conduct comprehensive health assessments, including physical exams and medical histories.

- Maintain accurate documentation in electronic medical records.
Key Responsibilities & Duties

Job Requirements
- Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
- Unrestricted New York State Nurse Practitioner license.
- DEA permit to prescribe medications within practice scope.
- Eligible to provide reimbursable services under Medicare and Medicaid.
- Minimum of 2 years of clinical experience, preferably in long-term care.
- Proficiency with eClinicalWorks (eCW) electronic health records.
- Strong knowledge of chronic care management practices.
- Excellent communication and collaboration skills.
- Ability to travel to multiple sites within a 30-mile radius.
