Overview
- Provide comprehensive care coordination and discharge planning for patients in an acute care setting.
- Collaborate with interdisciplinary teams to ensure seamless transitions and continuity of care.
- Utilize clinical expertise to advocate for patient and family needs during treatment and discharge processes.
- Perform utilization management activities to ensure appropriate resource allocation and adherence to medical necessity criteria.
- Maintain accurate and timely documentation within the Electronic Health Record (EHR) system.
- Support patients and families by providing education on diagnoses, treatment plans, and post-discharge care.
Key Responsibilities & Duties
- Develop, implement, and update individualized care plans tailored to patient needs.
- Coordinate discharge planning, including arranging home health services and durable medical equipment.
- Collaborate with physicians, nurses, and social workers to address barriers to care.
- Perform case management activities using Interqual criteria guidelines to ensure medical necessity.
- Advocate for patients and families, ensuring their understanding of treatment plans and post-discharge care.
- Maintain comprehensive and accurate documentation in compliance with organizational standards.
Job Requirements
- Valid Registered Nurse (RN) license in the state of California.
- Associate of Science (AS) in Nursing; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 1 year clinical experience; 3 years acute care experience preferred.
- Basic Life Support (BLS) certification required; Accredited Case Manager (ACM) or Certified Case Manager (CCM) preferred.
- Proficiency in critical thinking, communication, and organizational skills.
- Experience with Electronic Health Record (EHR) systems for accurate documentation.
