Job Description

Overview

  • Serve as the main contact for patients enrolled in HIV care coordination programs, fostering strong relationships to ensure engagement in prevention and treatment services.
  • Provide field-based case management services, addressing barriers to care and promoting optimal health outcomes for patients.
  • Collaborate closely with care teams, including medical providers and support staff, to deliver comprehensive patient care.
  • Support patients in navigating healthcare systems and accessing critical resources to enhance their health and well-being.
  • Maintain accurate client records and documentation in compliance with organizational protocols and standards.
  • Participate in quality assurance and continuous quality improvement initiatives to enhance service delivery.
  • Represent the organization at various venues to promote services and community engagement.
  • Perform additional tasks as assigned to support organizational goals and patient outcomes.

Key Responsibilities & Duties

  • Provide direct services to individuals living with HIV/AIDS under the supervision of the Care Coordinator.
  • Manage patient caseloads, including accompanying patients to appointments, delivering health education, and coordinating logistics for adherence reminders.
  • Observe and record patient self-administration of ARVs, assess and report side effects, and provide support in various convenient locations.
  • Maintain client-related records and documentation according to organizational standards and protocols.
  • Participate in quality assurance and continuous quality improvement tasks within the unit.
  • Represent the agency at community events and venues to promote services and outreach.
  • Collaborate with care teams to ensure comprehensive and coordinated patient care.
  • Perform other activities as assigned to support patient navigation and care coordination efforts.

Job Requirements

  • Associate’s degree in human services or social work, or equivalent certifications and training in patient navigation or case management.
  • Experience working with target populations, with fluency in one or more Asian languages considered a plus.
  • Strong written and verbal communication skills in English, with attention to detail and ability to handle multiple tasks.
  • Proficiency in common office software (Word, Excel, PowerPoint), with familiarity in eSHARE preferred.
  • Ability to work flexible hours, including evenings and weekends, and adapt to diverse populations and stressful environments.
  • Commitment to working within organizational structures and as part of a collaborative team.
  • Demonstrated ability to communicate effectively with medical providers and support staff.
  • Certification as a peer worker or equivalent training preferred, with socio-cultural identification with target populations advantageous.
