Job Description

Overview

  • Provide psychosocial support and social work services to hospice patients and families in various care settings.
  • Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to patient and family needs.
  • Support patients and families during pre- and post-bereavement phases of hospice service.
  • Serve as a liaison between hospice staff and inpatient/nursing home personnel.
  • Develop discharge plans and social work goals for patients in inpatient facilities.
  • Work primarily in Manhattan, providing field-based services with some weekend coverage.
  • Participate in virtual and in-person interviews as part of the hiring process.

Key Responsibilities & Duties

  • Develop and implement social work care plans for hospice patients in various settings.
  • Collaborate with interdisciplinary teams to address patient and family psychosocial needs.
  • Provide bereavement support and counseling to families during hospice care.
  • Coordinate with inpatient and nursing home staff to ensure seamless care transitions.
  • Maintain accurate clinical records and input data proficiently into electronic systems.
  • Offer supervision hours for LCSW licensure candidates.
  • Participate in interdisciplinary group meetings to review patient progress and care plans.
  • Ensure compliance with state licensure requirements and healthcare regulations.

Job Requirements

  • Master of Social Work degree from an accredited institution.
  • New York State LMSW or LCSW licensure is mandatory.
  • Minimum of one year of experience in a healthcare social work setting.
  • Proficiency in Microsoft Word and Excel is preferred.
  • Strong organizational, communication, and listening skills.
  • Ability to work effectively within an interdisciplinary team environment.
  • Experience in hospice or palliative care is highly desirable.
  • Fieldwork in Manhattan is required, with occasional weekend coverage.
