Overview
- Provide psychosocial support and social work services to hospice patients and families in various care settings.
- Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to patient and family needs.
- Support patients and families during pre- and post-bereavement phases of hospice service.
- Serve as a liaison between hospice staff and inpatient/nursing home personnel.
- Develop discharge plans and social work goals for patients in inpatient facilities.
- Work primarily in Manhattan, providing field-based services with some weekend coverage.
- Participate in virtual and in-person interviews as part of the hiring process.

Key Responsibilities & Duties
- Develop and implement social work care plans for hospice patients in various settings.
- Collaborate with interdisciplinary teams to address patient and family psychosocial needs.
- Provide bereavement support and counseling to families during hospice care.
- Coordinate with inpatient and nursing home staff to ensure seamless care transitions.
- Maintain accurate clinical records and input data proficiently into electronic systems.
- Offer supervision hours for LCSW licensure candidates.
- Participate in interdisciplinary group meetings to review patient progress and care plans.
- Ensure compliance with state licensure requirements and healthcare regulations.
Job Requirements
- Master of Social Work degree from an accredited institution.
- New York State LMSW or LCSW licensure is mandatory.
- Minimum of one year of experience in a healthcare social work setting.
- Proficiency in Microsoft Word and Excel is preferred.
- Strong organizational, communication, and listening skills.
- Ability to work effectively within an interdisciplinary team environment.
- Experience in hospice or palliative care is highly desirable.
- Fieldwork in Manhattan is required, with occasional weekend coverage.
