Job Description

Overview

  • Provide patient-centered cardiovascular care in a federally qualified health center setting.
  • Evaluate, diagnose, and manage cardiovascular conditions for an underserved community.
  • Collaborate with primary care providers to ensure comprehensive care coordination.
  • Perform cardiovascular assessments and interpret diagnostic tests.
  • Develop and manage treatment plans for acute and chronic cardiac conditions.
  • Educate patients on heart health, disease prevention, and medication management.
  • Document patient encounters accurately in the electronic health record system.
  • Participate in quality improvement initiatives and clinical meetings.

Key Responsibilities & Duties

  • Conduct thorough cardiovascular evaluations and provide accurate diagnoses.
  • Interpret diagnostic tests including EKGs, echocardiograms, and stress tests.
  • Formulate and implement effective treatment plans for cardiac conditions.
  • Collaborate with healthcare teams to deliver coordinated care.
  • Educate patients on cardiovascular health and preventive measures.
  • Maintain accurate and timely documentation in the electronic health record system.
  • Engage in clinical meetings and contribute to quality improvement efforts.
  • Support the mission of providing care to underserved populations.

Job Requirements

  • Medical degree (MD or DO) from an accredited institution.
  • Board certification or eligibility in Cardiology.
  • Current New York State medical license or eligibility for licensure.
  • Minimum of 1 year of experience in cardiovascular care; 4 years preferred.
  • Experience in outpatient or community health settings is advantageous.
  • Strong communication and patient engagement skills.
  • Proficiency in electronic health record systems.
