Overview
- Evaluate patients referred for hospice and palliative care services, ensuring compliance with regulations and policies.
- Provide clinical support and leadership for community and facility-based hospice programs.
- Facilitate patient transitions into hospice or palliative care services effectively and compassionately.
- Collaborate with interdisciplinary teams to deliver comprehensive care to patients and families.
- Engage in community outreach and education to promote hospice services.
- Participate in clinical marketing efforts to expand service areas and enhance program growth.
- Work flexible hours, including every other weekend, to meet patient care needs.
- Maintain accountability and uphold high standards of care in all interactions.
Key Responsibilities & Duties
- Evaluate and manage patient transitions into hospice or palliative care services.
- Coordinate interdisciplinary team resources to ensure comprehensive patient care.
- Deliver clinical support and oversight for hospice program development and expansion.
- Plan and execute community outreach initiatives to educate about hospice services.
- Provide direct care and support to hospice patients and their families.
- Collaborate with internal and external stakeholders to enhance clinical marketing efforts.
- Ensure compliance with applicable laws, regulations, and organizational policies.
- Maintain effective communication within a customer service-oriented environment.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of two years of experience in Medical-Surgical or Community Health nursing.
- Preferred experience in hospice or oncology nursing.
- New York State RN licensure and CPR-BLS certification preferred.
- Proficiency in Microsoft Office applications, including Word and Excel.
- Ability to work effectively within an interdisciplinary healthcare team.
- Strong communication, teaching, and critical thinking skills.
- Flexibility to work weekends and adapt to patient care needs.
