Job Description

Overview

  • Provide essential nursing care to individuals with mental health challenges, ensuring personalized support and treatment plans.
  • Collaborate with a multidisciplinary team to deliver holistic healthcare services in diverse community settings.
  • Conduct health assessments, monitor conditions, and manage medication plans tailored to client needs.
  • Educate clients and staff on preventive health practices and wellness strategies.
  • Coordinate care with external providers to ensure seamless treatment and support continuity.

  • Respond to medical emergencies and provide after-hours support as needed.

Key Responsibilities & Duties

  • Manage a caseload of clients, conducting regular visits and tailoring treatment plans to individual needs.
  • Administer medications, monitor effects, and ensure compliance with protocols.
  • Coordinate medication logistics, including delivery, refills, and insurance authorizations.
  • Facilitate referrals, schedule medical appointments, and provide accompaniment as needed.
  • Advocate for hospitalized clients, ensuring continuity of care and recovery support.
  • Conduct training sessions for staff and lead health education initiatives for clients.
  • Audit program documentation regularly to maintain compliance with care standards.
  • Supervise and mentor part-time nurses, overseeing their assignments and performance.
  • Actively participate in team meetings, representing medical perspectives and contributing to collaborative care strategies.

Job Requirements

  • Bachelor's degree in Nursing (BSN) and valid RN registration in New York State.
  • Minimum of one year of nursing experience, preferably in mental health or community settings.
  • Proficiency in medication management, health monitoring, and regulatory documentation practices.
  • Ability to work effectively in diverse community environments and adapt to field-oriented roles.
  • Strong communication skills for collaboration within multidisciplinary teams and client interactions.
  • Flexibility to provide after-hours emergency coverage as part of a rotational schedule.
