Overview
- Provide specialized care to hospice patients, including pediatric palliative care, through in-person and telehealth visits.
- Coordinate with interdisciplinary teams to ensure comprehensive patient care and symptom management.
- Conduct hospice eligibility evaluations and provide exigent medical care as required.
- Develop and implement teaching curriculum for pediatric hospice care.
- Participate in quality improvement projects to enhance care standards and practices.
- Utilize evidence-based approaches for patient management and policy development.
- Work collaboratively with team physicians and managers to optimize patient outcomes.
- Provide training and support to hospice team members on pediatric care aspects.
- Ensure compliance with best practices in history-taking, examination, care planning, and documentation.
Key Responsibilities & Duties
- Conduct face-to-face evaluations for hospice eligibility and manage exigent medical care for patients.
- Provide direct care to hospice patients in various settings, including homes, hospitals, and long-term care facilities.
- Assist in staffing pediatric palliative care consultation services as part of hospice programs.
- Coordinate with interdisciplinary teams to deliver holistic patient care.
- Develop and deliver training for hospice team members on pediatric hospice care.
- Participate in quality improvement initiatives to enhance care delivery.
- Utilize evidence-based practices for symptom management and care planning.
- Engage in effective communication with patients, families, and healthcare teams.
- Contribute to the development of policies and procedures for hospice care.
Job Requirements
- Master’s degree in nursing and completion of an approved Nurse Practitioner Program.
- Minimum of 2 years of experience as a Nurse Practitioner; 4 years preferred.
- New York State RN licensure and Nurse Practitioner certification required.
- CPR-BLS certification and valid NYS driver’s license preferred.
- Experience in hospice or palliative care settings strongly preferred.
- Proficiency in Microsoft Word and Excel for documentation and reporting.
- Strong skills in communication, teaching, and interdisciplinary team coordination.
- Ability to work non-judgmentally with diverse patient populations.
- Commitment to obtaining Hospice & Palliative Care certification within two years of hire.
