Overview
- Provide psychosocial support and social work services to hospice patients and families in various care settings.
- Develop and implement social work care plans tailored to patient and family needs.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Offer pre- and post-bereavement support to families during hospice services.
- Act as a liaison between hospice staff and inpatient or nursing home teams.
- Provide field-based services within the Bronx territory with some weekend coverage.
- Participate in virtual and in-person interviews as part of the hiring process.
- Utilize excellent organizational and communication skills to manage patient care effectively.
Key Responsibilities & Duties
- Develop and maintain social work care plans for hospice patients in various settings.
- Provide bereavement support and counseling to families during hospice care phases.
- Collaborate with interdisciplinary teams to address patient and family needs effectively.
- Coordinate discharge planning for patients transitioning from inpatient facilities.

Job Requirements
- Master of Social Work degree from an accredited institution.
- New York State LMSW or LCSW licensure is mandatory.
- Minimum of one year of experience in healthcare social work; four years preferred.
- Proficiency in Word, Excel, and clinical data input systems.
- Ability to work effectively within interdisciplinary teams.
- Strong organizational, communication, and listening skills.
