Hospice Social Worker

Overview

• Provide psychosocial support and social work services to hospice patients and families in various care settings.
• Develop and implement social work care plans tailored to patient and family needs.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Offer pre- and post-bereavement support to families during hospice services.
• Act as a liaison between hospice staff and inpatient or nursing home teams.
• Provide field-based services within the Bronx territory with some weekend coverage.
• Participate in virtual and in-person interviews as part of the hiring process.
• Utilize excellent organizational and communication skills to manage patient care effectively.

Key Responsibilities & Duties

• Develop and maintain social work care plans for hospice patients in various settings.
• Provide bereavement support and counseling to families during hospice care phases.
• Collaborate with interdisciplinary teams to address patient and family needs effectively.
• Coordinate discharge planning for patients transitioning from inpatient facilities.
• Serve as a liaison between hospice staff and external care teams to ensure seamless care.
• Document clinical data and patient progress proficiently using Word and Excel.
• Participate in training and supervision to maintain licensure and professional development.
• Provide field-based services with a focus on patient-centered care in the Bronx area.

Job Requirements

• Master of Social Work degree from an accredited institution.
• New York State LMSW or LCSW licensure is mandatory.
• Minimum of one year of experience in healthcare social work; four years preferred.
• Proficiency in Word, Excel, and clinical data input systems.
• Ability to work effectively within interdisciplinary teams.
• Strong organizational, communication, and listening skills.
• Willingness to provide field-based services in the Bronx territory.
• Commitment to professional development and maintaining licensure requirements.
