Job Description
Overview
- Provide multidisciplinary health services to patients within the community, ensuring comprehensive care and treatment plans.
- Collaborate with physicians, nurses, therapists, and social workers to develop and implement personalized care plans.
- Perform evaluations and assessments to monitor patient progress and adapt treatment plans accordingly.
- Supervise and evaluate aides and therapy assistants to maintain high-quality care standards.
- Document patient health status and progress using required forms and timelines.
- Participate in team conferences to ensure coordinated care for complex cases.
- Promote patient independence through education and tailored therapeutic interventions.
- Adhere to state regulations and agency policies while supervising therapy assistants.
Key Responsibilities & Duties
- Develop and implement individualized treatment plans based on comprehensive patient evaluations.
- Modify treatment plans and goals in response to patient progress and feedback.
- Complete OASIS assessments accurately and in accordance with established guidelines.
- Provide supervision and guidance to occupational therapy assistants and aides.
- Communicate effectively with patients, families, and healthcare team members.
- Monitor and report patient condition changes to appropriate personnel promptly.
- Participate in interdisciplinary team meetings to ensure comprehensive care coordination.
- Perform other duties as assigned to support patient care and organizational goals.
Job Requirements
- Master’s or Doctorate degree in Occupational Therapy from an accredited institution.
- Current, unencumbered license to practice Occupational Therapy in New York State.
- Minimum of one year of experience in occupational therapy preferred.
- Proficient in developing and implementing individualized treatment plans.
- Strong communication skills for effective interaction with patients and healthcare teams.
- Ability to supervise and evaluate therapy assistants and aides.
- Knowledge of OASIS assessment guidelines and documentation standards.
