Job Description

Overview

  • Provide multidisciplinary health services to patients within the community, ensuring comprehensive care and treatment plans.
  • Collaborate with physicians, nurses, therapists, and social workers to develop and implement personalized care plans.
  • Perform evaluations and assessments to monitor patient progress and adapt treatment plans accordingly.
  • Supervise and evaluate aides and therapy assistants to maintain high-quality care standards.
  • Document patient health status and progress using required forms and timelines.
  • Participate in team conferences to ensure coordinated care for complex cases.
  • Promote patient independence through education and tailored therapeutic interventions.
  • Adhere to state regulations and agency policies while supervising therapy assistants.

Key Responsibilities & Duties

  • Develop and implement individualized treatment plans based on comprehensive patient evaluations.
  • Modify treatment plans and goals in response to patient progress and feedback.
  • Complete OASIS assessments accurately and in accordance with established guidelines.
  • Provide supervision and guidance to occupational therapy assistants and aides.
  • Communicate effectively with patients, families, and healthcare team members.
  • Monitor and report patient condition changes to appropriate personnel promptly.
  • Participate in interdisciplinary team meetings to ensure comprehensive care coordination.
  • Perform other duties as assigned to support patient care and organizational goals.

Job Requirements

  • Master’s or Doctorate degree in Occupational Therapy from an accredited institution.
  • Current, unencumbered license to practice Occupational Therapy in New York State.
  • Minimum of one year of experience in occupational therapy preferred.
  • Proficient in developing and implementing individualized treatment plans.
  • Strong communication skills for effective interaction with patients and healthcare teams.
  • Ability to supervise and evaluate therapy assistants and aides.
  • Knowledge of OASIS assessment guidelines and documentation standards.
  • Commitment to promoting patient independence and health education.
